Hospital Discharge Transportation

Coordinated pickups from hospitals to home or to the next care setting, timed with the discharge process.

Who this service is for

  • Patients being discharged home
  • Case managers and discharge planners
  • Families coordinating a loved one’s trip home

Common trip examples

  • Hospital to home after an inpatient stay
  • Hospital to a rehabilitation or skilled-nursing facility
  • Emergency-department discharge to home (non-emergency transport)

What passengers can expect

Discharges rarely run on a fixed clock — so we coordinate directly with you, your family, or the discharge team, and adjust the pickup as the paperwork finishes. Your driver meets the patient at the designated discharge point, assists with belongings, and provides a calm, unhurried ride to the destination.

What we need to schedule

  • Hospital, campus and room or unit number
  • Expected discharge window
  • Destination address and entry notes
  • Facility or family contact for day-of coordination

Availability varies by day, vehicle type and service area. Submitting a request does not guarantee transportation until the trip has been reviewed and confirmed by Safety First Transportation.

We accept private pay and work with facility accounts. Insurance and payment options vary by plan and trip type. Verified payer list to be published
